We issue employment and salary certificates for individuals applying for retirement or disability pensions who were employed at our company. These documents are essential when applying for pension benefits and may be issued at the request of a former employee.

Copies and extracts from employee records

Process of Obtaining HR and Payroll Documentation

Step 1 – Download the Request Form
The request form can be downloaded from the Human Resources Department – Room 138 or from the downloadable materials section.
Step 2 – Prepare the Necessary Information
To complete the request, you will need the following data:
Full name, address, phone number, PESEL (personal ID number)
Job position, department, years of employment
Original documents issued by SRC S.A. (if available)
Step 3 – Fill Out the Form
Fill in the request form for HR and payroll documentation according to the instructions provided.
Step 4 – Submit the Form
Submit the completed application to the Human Resources Department in person or send it by mail to: Sanok Rubber Company S.A., Przemyska 24, 38-500 Sanok
An administrative fee may be required.
Step 5 – Wait for the Document
We will notify you when the document is ready for collection.
